Sounds like you either have low fluid levels, old worn out fluid, gummed up internals or a combo of any of them.
I would first check the fluid level and if it's low, just add some more to see if it helps. If it doesn't do much, I would drop the pan, replace the filter and refill with as much new fluid as you can. This will eliminate all the regular maintenance items out of your diagnostic equation.
